We are required to obtain your signature indicating that you have reviewed of the Notice of Policies and Practices of Lyles & Crawford, P.C. to Protect the Privacy of Your Health Information
Dr. Crawford Authorization Form (Adult)
This form is only required if you need Dr. Crawford to either release or obtain information regarding your medical history to a specific individual.
Dr. Crawford Authorization Form (Child)
This form is only required if you need Dr. Crawford to either release or obtain information regarding your minor's medical history to a specific individual.
This form is only required if you need Dr. Lyles to release information regarding your medical history to a specific indivdual.
